Why “prescribed disengagement” may be one of the most damaging messages given to people living with dementia There are phrases that describe a problem. Then there are phrases that expose it.“Prescribed disengagement” is one of those.The term was coined by Kate Swaffer, an Australian dementia advocate who was diagnosed with younger-onset dementia at 49. It describes what too often happens after diagnosis: a person is advised, explicitly or otherwise, to step back from work, avoid risk, abandon plans, prepare for decline and begin quietly withdrawing from the life they were living.The diagnosis is medical.The disappearance is socially prescribed.A recent article in The Guardian introduced several people living with dementia who have refused to follow that prescription. They continue to campaign, study, volunteer, participate in research, create organizations and challenge the peculiar notion that cognitive change erases personhood.They have been called “dementia rebels.”The fact that continuing to participate in one’s own life is considered rebellious tells us rather more about society than it does about dementia.The quiet shrinking of a life Prescribed disengagement does not always arrive as a blunt instruction from a doctor.Often, it enters the house wearing the very respectable coat of concern.Let me do that for you.Perhaps you shouldn’t go.That might be too much.You don’t need to worry about that anymore.We’ll make the decision.Each sentence may be well-intentioned. Some may even be necessary at particular times. But repeated often enough, they can steadily reduce a person’s world.Responsibilities disappear. Invitations become less frequent. Conversations take place around the person rather than with them. Ordinary mistakes are treated as evidence that nothing can be attempted safely again.Soon, the person is protected from almost everything except isolation, loss of confidence and the feeling that they are becoming a spectator in their own life.This is one of the crueler contradictions in dementia care. We rightly worry about what the disease may take away, then sometimes begin taking things away ourselves, years before the disease requires it.Ability is not an on-off switch Dementia is often presented as a smooth, inevitable descent from capability to incapability.Real life is considerably less tidy.A person may struggle with a task that once seemed simple, then do something complicated, creative or deeply familiar with ease. They may lose a word but retain the story. They may need help organizing the day but remain perfectly capable of contributing to it.Someone may be unable to work out how to move a car from an inconvenient place, yet still prepare a beautifully smoked salmon for a houseful of guests. He may need prompting in one moment and spend the next two hours talking, laughing and exchanging stories with people around him.Both things can be true.That unevenness can be confusing for families. We naturally look for a clear line between “can” and “cannot,” because a line would make decisions easier.Dementia does not have the courtesy to provide one.The better question is not simply, “Can this person still do this alone?”It is, “What support would allow this person to continue doing as much of it as possible?”That small change in language moves us from replacement to adaptation. It preserves ability instead of waiting for failure. Most importantly, it keeps the person involved.Safety matters. So does agency. None of this means ignoring genuine risk.There will be decisions about driving, medication, finances, cooking, wandering and personal safety that cannot be wished away by positive thinking and an inspirational quotation laid over a photograph of a sunset.Dementia is real. Risk is real. Decline is real.But autonomy and safety are not always enemies.There is a difference between a reckless risk and the ordinary risks every human being takes by remaining alive. We drive, cook, travel, fall in love, try unfamiliar things, make imperfect decisions and occasionally make an absolute hash of something. That is not evidence of pathology. It is the untidy business of being a person.The goal should not be to eliminate every conceivable risk until nothing resembling life remains.It should be to understand the particular risk, add appropriate support and preserve as much choice as possible.A reminder can support medication without removing control.A prompt can help someone complete a familiar task without completing it for them.A shared plan can provide structure without turning the day into a set of instructions issued by someone else.A check-in can offer reassurance without becoming surveillance.Good support should function like a handrail, not a locked door.This is why Memory in Motion exists Memory in Motion was not created because technology can defeat dementia. It cannot.It was created because there is an enormous space between diagnosis and dependency, and that space is too often treated as though it barely matters.It matters enormously.It contains breakfasts and appointments. Stories and shopping lists. Familiar music. Family jokes. Walks with the dog. Forgotten words and remembered recipes. It is supported agency: helping someone continue to choose, contribute and participate, even when the way they do those things begins to change.Do not confuse preparation with surrender Families should prepare. Legal documents matter. Financial arrangements matter. Difficult conversations matter.But preparation for what may come must not consume the life that is happening now.A diagnosis may change the route. It may change the speed. It may eventually change who must hold the map.It does not mean the journey is over.People living with dementia do not need to be wrapped in fear and placed carefully on a shelf. They need truthful information, practical support, patience, adaptation and the dignity of remaining part of the world.They need opportunities to speak, decide, learn, help, laugh, disagree and occasionally get things wrong.In other words, they need what the rest of us need.A dementia diagnosis may require adaptation.It should never become an eviction notice from life.

<h3>Why I’m Writing About This?</h3>I came across a paid article on Substack this week titled Pharma’s Influence on the FDA. I did not subscribe, partly because I already have enough subscriptions quietly nibbling at my bank account like highly educated mice, and partly because the subject itself is bigger than one article.But the title stopped me.Because if you are living anywhere near Alzheimer’s, dementia, early diagnosis, biomarker testing, infusion drugs, clinical trials, or the exhausting emotional weather system that surrounds all of it, the relationship between pharmaceutical companies and the FDA is not an abstract policy debate.It is personal.It sits inside the questions families are being asked to answer.Should we pursue this drug?What does approval really mean?How much benefit is proven?What are the risks?Who paid for the research?Who reviewed the evidence?Who gets to call something “meaningful”?And who benefits when desperate families are told there is finally hope?I am not anti-pharma. That would be foolish. Modern medicine has given us extraordinary treatments, and many people are alive today because pharmaceutical research worked. I am also not anti-FDA. The agency has an almost impossible job, standing between public safety, political pressure, scientific uncertainty, industry urgency, and patients who do not have the luxury of waiting forever.But I am deeply wary of the phrase “nothing to see here.”Nothing to see here is what powerful systems say when the public starts looking too closely.Nothing to see here is what gets said when conflicts are technically disclosed but not truly understood.Nothing to see here is what families hear when they ask perfectly reasonable questions and are made to feel difficult, emotional, or uninformed.And in Alzheimer’s, where fear and hope are already pressed so tightly together they can barely breathe, “nothing to see here” is simply not good enough.We need better than blind trust.We need better than blanket suspicion.We need transparency, humility, and plain English.Because approval is not the same thing as certainty. Biomarker change is not the same thing as restored life. Hope is not the same thing as proof. And families deserve to know where the evidence is strong, where it is still evolving, and where commercial pressure may be standing just a little too close to the microphone.So this is not a conspiracy piece.It is a trust piece.And trust, once it starts leaking, is very hard to patch with press releases.

Fact: Dementia is not a normal part of aging.Yes, memory can change as we get older. We may forget a name, walk into a room and wonder what on earth we came in for, or misplace the glasses currently sitting on our head.That is one thing.Getting lost on a familiar route, struggling with bills, losing the thread of ordinary routines, having major changes in judgment, or finding once-simple tasks suddenly confusing is something else.Dementia is caused by diseases that affect the brain.It is not simply “getting old.”Myth: Memory loss is the only symptom. Fact: Dementia can affect far more than memory.Memory may be part of the picture, but dementia can also affect judgment, language, planning, confidence, mood, behavior, spatial awareness, problem-solving, and the ability to manage daily life.This matters because families often miss the early signs when they are only looking for forgetfulness.Sometimes the first clues are not “I forgot.”Sometimes they are:Why is this task suddenly overwhelming?Why does this familiar place feel confusing?Why is decision-making harder?Why is the person more anxious, withdrawn, irritable, or unsure?Why does Tuesday morning now require a committee?Memory matters.But daily function tells a much bigger story.Myth: A good screening score means everything is fine. Fact: Some people test well and still struggle in real life.A screening test is useful. It can be an important clue.But it is not the whole person.Some very bright people can perform well on cognitive screening tests while still struggling in daily life. They may compensate. They may mask. They may have learned how to navigate tests beautifully while falling apart at the pharmacy, the stove, the calendar, or the car keys.A test score is not a marriage.It is not a kitchen.It is not a pill organizer.It is not a familiar road in bad weather.It is not a Tuesday afternoon when everyone is tired and the dog needs feeding.Daily function matters.Patterns matter.What families observe at home matters.Myth: Every medical professional fully understands dementia testing, APOE status, biomarkers, and what the results actually mean. Fact: Many are excellent. Some are not current. Some explain complex risk badly.This is not a criticism of every doctor, nurse practitioner, specialist, or medical team.It is a plea for better questions.Dementia is complicated. Alzheimer’s disease is complicated. Biomarkers are complicated. Genetics are complicated. Differential diagnosis is complicated.And sometimes families are handed complex information in a way that sounds more final than it really is.Ask questions.Ask what the test shows.Ask what it does not show.Ask whether the diagnosis fits the symptoms, function, history, medications, sleep, mood, vascular health, imaging, biomarkers, and the person’s real life.Ask what else could be contributing.Ask whether anything treatable has been ruled out.A confident sentence is not the same as a complete evaluation.Myth: APOE4 is a death sentence. Fact: APOE4 can increase Alzheimer’s risk, but risk is not destiny.APOE4 is an important genetic risk factor. It may raise risk for some people, especially those with two copies.But it does not decide the whole future.Some people with APOE4 do not develop Alzheimer’s. Some people without APOE4 do.Genes can load the dice.They do not play the entire game.Also, for the record, we are all technically terminal.That is called being alive.The goal is not to panic because a gene showed up wearing a black cape. The goal is to understand risk, ask better questions, and keep living with as much clarity, agency, humor, and support as possible.Myth: Amyloid plaque automatically means dementia. Fact: Amyloid plaque has been found in people who never developed dementia.That does not mean amyloid is irrelevant.It can matter. It may increase risk. It may be part of the Alzheimer’s disease process.But finding amyloid is not the same as saying, “Case closed, courtroom adjourned, everyone go home.”Brains are complicated.Annoyingly so.A biomarker is a clue. Sometimes a very important clue.But it still has to be interpreted alongside symptoms, function, history, imaging, medications, sleep, mood, vascular health, and the person’s real life.The human being is not a lab result with shoes on.Myth: FDA-approved means simple, settled, and right for everyone. Fact: FDA-approved means a treatment met the agency’s standard for approval for a specific use in a specific population.It does not mean the treatment is right for every person.It does not erase risk.It does not end the conversation.It begins a much more serious one.Families need to know who the treatment was studied in, what benefit was shown, what risks exist, what monitoring is required, what the burden looks like, what the costs may be, and whether the treatment aligns with the person’s values and goals.“Approved” is important.It is not a magic wand.Myth: Infusions are a cure. Fact: Current anti-amyloid infusion treatments are not cures.For some carefully selected people in the early stages of Alzheimer’s disease, certain anti-amyloid treatments may slow decline.That is not nothing.But it is also not reversal.It is not a cure.It is not right for everyone.These treatments require careful diagnosis, biomarker confirmation, monitoring, repeated appointments, and a very honest discussion about risks, burden, benefit, and goals.Families deserve that sentence without glitter on it.Myth: Over-the-counter supplements are harmless because they are “natural.” Fact: Natural does not mean safe, effective, or proven.This one needs to be said plainly.A supplement bottle is not evidence.A label is not a clinical trial.A podcast recommendation is not a treatment plan.Supplements can interact with medications. They can delay proper care. They can drain families financially. They can offer just enough hope to make a person feel that if they are not buying the bottle, they are not doing enough.That is a cruel little trap.Some supplements may be appropriate for some people in some circumstances.But “natural” should never be confused with safe, effective, or necessary.Myth: Dementia can be reversed if you find the right protocol, product, clinic, supplement stack, or miracle plan. Fact: For Alzheimer’s and other progressive dementias, reversal is not currently proven.There is no cure yet.That is the sad fact.Some treatments may slow decline for some people. Some medications may help symptoms for a period of time. Lifestyle changes, sleep, movement, nutrition, social connection, music, routine, and reducing stress may support quality of life and daily function.But slowing, supporting, stabilizing, or improving daily life is not the same as reversing the disease.Words matter.Because when frightened families hear “reversal,” they may spend money, time, hope, and emotional energy chasing a promise science has not yet earned.Hope is allowed.False certainty is not.There is one important caveat: some dementia-like symptoms can improve if they are caused by something else, such as medication effects, vitamin deficiency, infection, depression, thyroid problems, sleep issues, or other treatable conditions.That is exactly why the diagnosis has to be careful.Before anyone declares the future, they need to understand the whole picture.Myth: Once someone is diagnosed, symptoms suddenly speed up. Fact: Diagnosis does not magically press the accelerator.The disease process was already underway before anyone named it.What often speeds up is everyone else’s fear.Suddenly every forgotten word becomes decline.Every misplaced item becomes proof.Every hesitation becomes a family meeting.Diagnosis can change how we see the person.That does not mean the person changed overnight.Myth: After diagnosis, we should lock everything down immediately. Fact: Safety matters. Panic is not a care plan.Taking away the keys, the stove, the walks, the errands, the privacy, the choices, the friendships, and the ordinary rhythms of life in one dramatic sweep may feel protective.It may also feel, to the person diagnosed, like being erased while still standing in the room.The better question is not:What can we take away?The better question is:What can we support safely?Can we simplify the route?Can we use reminders?Can we add location sharing?Can we cook together?Can we set up medication systems?Can we create routines?Can we adjust before we confiscate?There may come a time when some things truly are no longer safe.But fear should not be allowed to make every decision in advance.Myth: Stigma is not a big deal anymore. Fact: Stigma is still a very big deal.Stigma makes people hide symptoms.It makes families delay conversations.It makes people avoid diagnosis.It makes friends disappear because they do not know what to say.It makes community shrink at the exact moment people need connection.Stigma does not protect anyone.It just makes the room smaller.Myth: If we ignore it, maybe it will go away. Fact: Ostrich syndrome is human. It is not a treatment plan.I understand denial.Truly.There are moments when denial feels like the only available chair in the room.But dementia is not especially impressed by sand.Putting our heads down may feel safer for five minutes. It does not build support. It does not create routines. It does not clarify risk. It does not protect independence. It does not prepare the family.Avoidance is understandable.It is not strategy.Myth: Social media groups always make families better informed. Fact: Social media groups can be helpful, generous, and lifesavingly kind.They can also become fear amplifiers.And I say that with sympathy, not judgment.When people are frightened, they turn to each other. Of course they do. Humans are not built to sit alone in terror with a search bar and a diagnosis code.But fear has a way of creating momentum.One person posts a terrifying story.Another shares a miracle claim.Someone recommends a supplement, a protocol, a test, a clinic, a specialist, an infusion, a diet, a scan, a genetic interpretation, or a dramatic intervention that “changed everything.”And suddenly the group begins moving as one.Not because people are foolish.Because they are scared.Because they love someone.Because uncertainty is unbearable, and certainty, even the fake kind, feels like a handrail.That is how anecdote becomes evidence.That is how panic becomes advice.That is how one family’s experience becomes a rule for everyone else.Social media can give families language, support, validation, and community.But it should not become your neurologist, pharmacist, genetic counselor, research department, and treatment plan wearing a cardigan in the comment section.Use the groups.Learn from people.Listen to lived experience.But do not let collective fear make your decisions for you.Myth: Music is just a nice extra. Fact: Music can be powerful.Familiar music can cue memory, emotion, language, movement, connection, and identity.Sometimes the right song reaches a place ordinary conversation cannot quite get to.No, music is not a cure.But it can be a key.And when a key opens a door, we should stop calling it “just entertainment.”Myth: There is nothing useful to do after diagnosis. Fact: There is a great deal to do.This may be the most important one.Families are often told what is happening, but not what to do on Tuesday morning.How to manage routines.How to preserve independence safely.How to reduce panic.How to support identity.How to avoid turning fear into control.How to make the home, the day, and the relationship work after diagnosis.This is where so many families are left alone.Diagnosis should open the door to practical, human, daily-life support.Not simply hand people a label and a leaflet.So what can we do? We can stop treating diagnosis like a trapdoor.We can ask better questions before accepting terrifying answers.We can get second opinions when something does not feel right.We can look at the whole person, not just one test score, one scan, one biomarker, one gene, or one rushed appointment.We can build support around daily life.Not someday.Now.We can create routines that make mornings easier.We can use calendars, notes, labels, reminders, playlists, smart speakers, and simple checklists.We can simplify choices without removing choice.We can reduce clutter, friction, noise, and unnecessary stress.We can protect sleep, movement, food, hydration, connection, and purpose.We can make medication safer.We can write down what works while it still works.We can preserve stories, preferences, music, rituals, recipes, routes, jokes, and the small familiar things that make a person feel like themselves.We can stop confusing support with takeover.We can stop confusing safety with imprisonment.We can stop waiting for crisis to become the project manager.And we can remember this:The goal is not to pretend nothing has changed.The goal is to support what still remains.Because there is still life after diagnosis.There are still walks to take.Songs to play.Dogs to feed.Grandchildren to laugh with.Gardens to fuss over.Meals to cook.Stories to tell.Errands to manage with support.Golf balls to hit badly, if necessary.Tea to drink.And entire ordinary Tuesdays worth protecting.The gap after diagnosis Dementia is medical.But living with dementia is deeply human.Families do not need more panic, shame, miracle claims, vague reassurance, or sales pitches in lab coats.They need honest information.They need practical tools.They need better questions.They need support that begins after diagnosis, not only after crisis.That is the gap MiM Memory in Motion is being built to address.The daily life gap.The identity gap.The place where families are too often told, “Come back when it gets worse,” when what they need is help making today work better.Fear creates fictitious facts.Clarity gives families something solid to stand on.And after diagnosis, that matters.A note from MiM MiM Memory in Motion is not medical care and does not diagnose, treat, or cure dementia.MiM is being built to support daily life after diagnosis by helping families preserve routines, connection, identity, and practical support where it matters most:In the kitchen.In the car.On the walk.At the pharmacy.By the speaker playing the song that somehow still knows the way home.

<h3>Music Is Not Background Noise When Memory Begins to Change</h3> This week, I had a conversation with Dan Cohen, founder of Music &amp; Memory and Right to Music, and the subject of the Sundance award-winning documentary Alive Inside.I reached out to Dan because MiM Memory in Motion is being built in the space after diagnosis but before traditional “care” begins.That space is far too empty.Families are often given warnings, pamphlets, risk language, medication discussions, future planning advice, and sometimes a rather bleak tour of everything that may go wrong.What they are not always given is a way to keep daily life connected.A way to preserve identity.A way to support the person who is still very much here.That is where music belongs.Not as entertainment.Not as a pleasant add-on.Not as something nice to do if there is time after all the serious things have been handled.Music may be one of the serious things.Dan said something during our conversation that stopped me in my tracks.The power of music is not really about genre.It is about familiarity.That sounds simple, but it changes everything.It is not about whether someone liked classical music, country music, hymns, jazz, Motown, the Beatles, Welsh choirs, marching songs, disco, opera, or whatever was playing in the car during the summer of 1978.The question is not, “What kind of music did they like?”The better question is, “What music was theirs?”What song did they dance to?What song played at their wedding?What did they sing in school?What did their mother hum in the kitchen?What was on the radio when they were driving to work, falling in love, leaving home, raising children, surviving grief, finding themselves, or becoming themselves?That music is not background noise.It is an access point.It is attached to story, rhythm, place, identity, and emotional memory.And here is the part that matters deeply for MiM.We need to ask these questions early.Not when the person is already in late-stage disease.Not when the family is exhausted and trying to reconstruct a life from fragments.Not when everyone is standing around saying, “I wish we had known.”We need to ask while the person can still tell us.Because too often, families do not know.A daughter may know her mother’s medical history, medication list, insurance details, and appointment schedule, but not the songs that made her mother feel most alive.A spouse may know every pharmacy refill and still not know the music that unlocks a memory.That is not a failure of love.It is a failure of systems.Our health and care systems are not built around the preservation of personhood. They are built around diagnosis, risk, crisis, billing codes, referrals, and eventually care plans.But life is not a care plan.Life is mornings, habits, smells, music, jokes, arguments, recipes, walking routes, favorite chairs, family sayings, songs in the car, and the thousand tiny rituals that tell a person: this is me, this is mine, I am still here.That is why MiM exists.MiM is not medical care. It does not diagnose, treat, or promise to slow disease. It is not here to pretend dementia can be solved with a playlist and a positive attitude. That would be insulting, and frankly, dangerous.But MiM is here to take daily life seriously.To support the gap between diagnosis and dependency.To help Travelers and Supporters build practical scaffolding around identity, routine, movement, connection, safety, and memory.And music belongs in that scaffolding.Not vaguely.Not as “play something cheerful.”Not as “put on oldies.”Personalized music has to be personal.Dan talked about the danger of settling for a weak response and calling it success. If someone seems calmer when generic music is playing, that may be useful. But it may also be only a fraction of what is possible if the music is truly familiar, truly connected, truly theirs.That distinction matters.Because we do this in dementia care all the time.We accept less.A person smiles once, and we call it engagement.A person sits quietly, and we call it calm.A person stops objecting, and we call it success.But what if the real goal is not just reduced distress?What if the goal is recognition?Connection?Agency?Pleasure?A moment of self returning to the surface?There is a cruelty in assuming that because someone has memory loss, the details no longer matter.The details may matter more than ever.The right song is not magic.But it may be a bridge.And bridges matter when the terrain is changing.One of the clearest lessons from my conversation with Dan is that we should not wait. We should not wait until the crisis. We should not wait until the move. Less themselves.And because dementia is already in the room, everyone looks at the diagnosis first.Of course they do.The diagnosis becomes the loudest suspect.But sometimes the diagnosis is not acting alone.This is where families often notice what the system misses.Not because families know more medicine. They usually do not. Not because they should be expected to become unpaid pharmacists at the kitchen table. That way lies madness, and possibly a three-ring binder with tabs.Families notice because they live in the pattern.They know what Tuesday morning usually looks like. They know whether Dad has always needed a nap after lunch or whether this is new. They know whether Mum has always been a little anxious before appointments or whether she is now pacing the hallway at 4 p.m. every day. They know when the person they love has gone subtly off-kilter.That kind of noticing matters.A medication list should not just answer, “What is being taken?”It should help answer:What changed? When did it change? Who changed it? Why was it changed? What happened afterward? They capture a version of the truth, but not necessarily the living truth.The list may say the medication is taken daily.But is it?The list may say the dose is 10 mg.But is the bottle still 5 mg because the old refill is being used up first?The list may say one medication was discontinued.But is it still sitting in the weekly pill box because nobody knew it had been stopped?The list may include prescriptions.But does it include the antihistamine, the sleep aid, the pain reliever, the CBD gummy someone recommended, the magnesium powder, the “memory supplement,” the herbal tea, the old antibiotic in the bathroom cabinet, the occasional glass of wine, or the pill borrowed from a spouse because “it worked for them”?This is not a lecture. It is a reality check.Medication does not happen in a spreadsheet. It happens in a house.It happens with refill delays, rushed appointments, tiny labels, different pharmacies, changing routines, and people trying very hard to stay independent.For someone living with early cognitive change, the gap between the official medication list and the real-life medication routine can widen quietly. A pill taken twice because it was forgotten. A pill skipped because the morning got interrupted. A new side effect mistaken for decline. A decline assumed to be inevitable because the word Alzheimer’s has entered the room and started throwing its weight around.That is the danger.Once a diagnosis is attached to a person, normal questions sometimes stop being asked.Is this confusion new? Did it begin after a medication change? Did sleep change first? Did balance change first? Did appetite shift? Did agitation appear after the dose increased? Did the “bad week” begin after the hospital visit? Did anyone compare the bottles to the portal list? Did anyone ask the pharmacist?This is where MiM’s way of thinking becomes useful.We are not trying to turn families into clinicians.We are trying to preserve the everyday record.Because the everyday record is often where the clues live.A Supporter may not know whether a medication is causing a symptom. That is for a qualified medical professional to assess. But a Supporter can notice that the symptom began three days after a change. A Traveler may not remember whether they took the new pill at breakfast, but the household routine may show that the pillbox was opened twice. A family may not know what the answer is, but they can bring better questions.And better questions can change the appointment.Instead of saying, “He seems worse,” a family can say:“He became much sleepier after the evening medication was added.”“She started feeling dizzy the week after the dose changed.”“He is more confused on the mornings after poor sleep.”“She has been taking an over-the-counter sleep aid we did not realize mattered.”“The pharmacy list, hospital discharge list, and pill bottles do not match.”That is not medical advice.That is evidence.And evidence is precious.The practical step is simple, though not always easy: build a living medication record.Not just the name of each medication, but the real-world context around it.Name. Dose. Time of day. Reason it was prescribed. Prescribing clinician. Pharmacy. Start date. Stop date. Dose changes. Side effects noticed. Supplements. Over-the-counter medications. Missed doses. Duplicate bottles. Old prescriptions still in circulation. Questions for the doctor or pharmacist.Also note the human part.Sleep. Appetite. Mood. Balance. Bathroom changes. Alertness. Agitation. Confusion. Falls. New fears. New routines. New resistance. New fatigue.Not obsessively. Not with the joyless energy of a tax audit.Just clearly enough that patterns have somewhere to land.Because what looks like “just getting worse” may sometimes be something else.Something adjustable. Something explainable. Something worth asking about before everyone surrenders to the diagnosis.The medication list is not the enemy.But an unexamined medication list can become a place where the story gets lazy.And in early dementia, lazy stories are dangerous.They flatten the person.They make every change look inevitable.They give the diagnosis too much power and the family too little information.So no, families should not stop medications on their own. They should not play doctor. They should not go hunting through side-effect lists at midnight until the internet turns into a haunted forest.But they can observe.They can document.They can compare.They can ask.They can bring the quiet crime scene into the light and say, kindly but firmly:“Before we call this decline, can we please look at the whole list?”That question may not solve everything.But it can open a door.And sometimes, in the long corridor after diagnosis, an open door is no small thing.

The Things We Miss Because They Look Normal

Not because they are irresponsible people wandering around the house like unlicensed emotional electricians.They wait because the conversation feels too early.They wait because it sounds dramatic.They wait because Mum is fine.They wait because Dad will be offended.They wait because no one wants Sunday lunch to become a small courtroom with mashed potatoes.And then something happens.A fall. A hospital stay. A diagnosis. A sudden change in memory. A bank account no one can access. A doctor who cannot speak freely. A sibling who thought someone else had handled it. A parent who can no longer clearly understand the document that everyone now urgently needs to be signed.That is when families discover the truth.The right time was not when everyone felt ready.The right time was when the person could still choose.<h2>The paperwork is not the point.</h2> Power of attorney is often discussed as a legal document, which, of course, it is. Families should speak with a qualified elder law attorney or other appropriate legal professional in their area, because laws vary by state and country.But the deeper issue is not paperwork.The deeper issue is agency.A power of attorney conversation, done early, is not about taking control away from someone. It is about making sure that if help is ever needed, the person who steps in is someone they chose, while they were still fully able to make that choice.That distinction matters.Because many older adults hear “power of attorney” and immediately think “takeover.”They hear: You do not trust me.They hear: You think I am losing it.They hear: you are quietly moving me from the driver’s seat to the back seat while pretending it is about “planning.”No wonder they resist.Families often make this worse by arriving at the conversation with a nervous expression, a folder of documents, and the emotional subtlety of a forklift.The better conversation begins somewhere else.Not with, “You need to sign this.”But with “If something happened and you needed help, who would you want to speak for you?”That is not a takeover.That is a choice.<h2>Capacity is the quiet deadline.</h2>The hard part is that the window does not announce itself when it starts to close.Capacity is not the same as being cheerful at dinner. It is not the same as remembering a grandchild’s name, telling a funny story, or insisting with great authority that nothing is wrong.A person can seem mostly fine in ordinary conversation and still struggle with complex decisions, documents, finances, medications, risk, or consequences.A diagnosis of mild cognitive impairment or early Alzheimer’s does not automatically mean someone has lost capacity, far from it. Many people remain deeply capable of expressing their wishes, naming who they trust, and participating meaningfully in planning.But it does mean the calendar has changed.Not because the person has disappeared.Because the person is still here.That is the whole point.The earlier conversation is not an insult. The answer is to change the frame.This is not about declaring someone incapable.This is about asking what they want while they are capable.This is not about taking control.This is about building a backup.A spare key is not an eviction notice.A power of attorney is not a verdict.A healthcare proxy is not a surrender.These documents, when done properly and early, can become part of a person’s own plan for staying respected, protected, and heard.<h2>The conversation before the corridor</h2> Hospital corridors are terrible places to make first-time family decisions.So are emergency rooms.So are bank lobbies.So are conference calls between siblings who have not agreed on anything since 1987.Crisis turns every conversation louder. It compresses time. It sharpens fear. It turns small family fault lines into tectonic events.The goal is not to eliminate every future difficulty. Life is not that tidy, and families are not software updates.The goal is to avoid making the first conversation happen when everyone is exhausted, frightened, and legally stuck.A calmer conversation might begin with one question:“If something happened and you needed help for a while, what would you want us to know?”Then another:“Who would you trust to speak for you if you could not speak for yourself?”And another:“Do you already have documents in place, and do we know where they are?”And another:“Would you be willing to meet with an elder law attorney or legal professional now, while this is still planning and not panic?”These are not easy questions.But they are kinder than silence.<h2>Why this belongs in the early stage</h2> At MiM, we spend a lot of time talking about the gap after diagnosis.That strange, under-supported space where someone is not in crisis, not in late-stage care, not “gone,” not helpless, and yet daily life has started to change.This is where so much gets missed.Families are often told to monitor, wait, come back in six months, reduce stress, keep routines, and “plan.”But what does that actually mean on a Tuesday?It means having the conversation before the crisis.It means noticing that early memory change is not only a medical issue. A timing issue.It means understanding that support does not have to begin with an emergency.Support can begin with a conversation.<h2>The question is not “Are we there yet?”</h2> Families often ask: when is the right time?The better question is: what are we waiting for?If everyone is healthy, talk now.If there has been a diagnosis, talk now.If there has been a fall, a hospitalization, or a close call, talk now.If one parent manages everything and the other has no idea where anything is, talk now.If siblings are likely to disagree later, talk now.If your parent lives alone, talk now.If the thought of having the conversation makes your stomach clench, that may be exactly why it matters.The right time is not when the conversation becomes comfortable.The right time is when the person can still participate clearly and freely.<h2>Choice is easier to preserve than recover</h2> There is a painful irony in waiting too long.Families often delay these conversations out of respect. They do not want to upset the person.
